It is part of an informal effort by nursing programs at area schools to coordinate efforts to ensure that more nurses are being produced at all levels.
Augusta Tech has wanted a nursing program for years. But just in the past year it completed a study showing not only a need but also that the school will be able to count on all of the local hospitals as clinical teaching sites, President Terry Elam said.

"The big thing was could we supply the clinical sites," he said.

University Hospital was a key piece of that, Mr. Elam said. The school is aiming for 25 students a class in the two-year program, which would allow them to become registered nurses, he said. Many of those will go on to a four-year program, either at Medical College of Georgia or perhaps Augusta State University, which nursing leaders said is hoping to convert its two-year program into a four- year baccalaureate program.

In addition, MCG is pushing for more graduate education of nurses. Its new Clinical Nurse Leader program allows those with other undergraduate degrees to get a master's degree in nursing.

"We did establish our philosophy that we see different roles for the different institutions in this area, but there will be overlapping also," said Lucy Marion, the dean of the MCG School of Nursing.

The school will continue its undergraduate baccalaureate program but supports Augusta State getting one "because we support the highest-educated nurse to meet the needs of this very complex health society," she said.

Overall, the University System of Georgia plans to increase nursing graduates from 1,900 last year to 2,700 by 2010. That means the state would be producing more than 3,000 new nurses a year.

"That's still not enough," Dr. Marion said.

The Georgia Hospital Association found in 2006 that 59 percent of the nurses in the state were over age 40 and 27 percent were older than 50 . That means the state will need at least 8,000 more nurses by 2012, according to the Nursing Education Task of the University System of Georgia.

That's why University is supporting all of these efforts, said Marilyn Bowcutt, the vice president for patient care services.

"We know that we have to start getting more people in the pipeline in order to replace the retiring nurses, for the future," she said.

Inc., 1247 Milwaukee Ave., Suite 303, Glenview, IL 60025, USA.

<p class="MsoNormal" dir="ltr" style="MARGIN-BOTTOM: 0pt; With $450,000 from the Golden LEAF Foundation, the college and hospital will turn the former Wilson-Greene Mental Health Center on the hospital campus into a lab for nursing students. State-of-the-art mannequins will simulate patient symptoms, reactions and behaviors, giving nursing students safe and realistic opportunities to practice their skills.

The Golden LEAF funds will be used to purchase seven high-tech mannequins and three more mannequins that are not quite as sophisticated. The former Mental Health Center will be remodeled as a hospital ward with patient rooms, a nursing station and a simulated environment for students. Cameras will record learning sessions so that procedures can be reviewed and critiqued.

The aim of this laboratory and other efforts to encourage nursing students is the national shortage of registered nurses. Nurses have been in short supply for several years with crises arising periodically in various locations.

There are many reasons for the nursing shortage, which shows little sign of disappearing:

--A study earlier this year estimates that the nation's nursing shortage will increase to 340,000 by 2020.

--A 2006 study found that 55 percent of registered nurses, including a majority of nurse managers, intend to retire between 2011 and 2020.

--Although employment of registered nurses in hospitals increased by 185,000 between 2001 and 2004, more than 80 percent of physicians and nurses thought their hospitals told a poll they thought there was a nursing shortage where they worked.

--Nurses are older as fewer young people enter the profession. In March 2004, the average registered nurse was 46.8 years old, and only 8 percent of the nursing population was under 30.

--Nurses are essential to health care. They are the first line of contact with patients and are expected to handle increasingly sophisticated and complicated procedures. Without an adequate supply of good nurses, hospitals and other medical facilities cannot function.

The effort by Wilson Medical Center and Wilson Community College should help ease the nursing shortage by improving training of student nurses. Golden LEAF or another philanthropy might also consider scholarships for nurses who will agree to work in underserved rural areas, where recruiting medical professionals can be difficult.

Inc., 1247 Milwaukee Ave., Suite 303, Glenview, IL 60025, USA.

Unknownnoreply@blogger.com0tag:blogger.com,1999:blog-503732791294005663.post-25628063711440795162008-04-13T06:32:00.000-07:002008-04-13T13:13:02.935-07:00Training Nurses

Training nurses

Times of London 03-07-2008

Training nurses
Edition: Final 1
Section: Features
Type: Letter

Sir, I write in support of Lord Mancroft's comments on his nursing care while in hospital (report, March 1). I was the head of a large nurse training school until I retired in 1989 and had the misfortune to be admitted for nearly a month to a medical ward. I could not believe the poor personal care I received. It was so bad that after a week I made a formal complaint, and things improved somewhat. When discharged I made a further formal complaint and received an apology.

However, this did not address the root cause - the absence of qualified nurses giving personal care, and the total absence of senior nurses with management titles such as "quality control" in clinical areas. In addition, trainee nurses are placed on wards and left to work with nursing assistants with no one teaching them the art of nursing. Nursing is a practice-based discipline and cannot be taught in institutes of higher education by teachers who never go near clinical areas, and haven't for years.
What is required is an inquiry by the Department of Health and the statutory body with input from the Royal College of Nursing into the current model of nurse management of patient care in hospital and a suitable replacement.

B.E. After the war the volunteers spearheaded a movement for formal nurses' training. Simultaneously, hospitals also recognized a need for more skilled, disciplined, and competent nurses to improve the quality and efficiency of care.

By the early twentieth century women wishing to nurse "bartered" two or three years of service to hospitals in exchange for a diploma that promised entry into a respectable, autonomous form of work, free from the constraints that accompanied other "women's" occupations, such as teaching, factory labor, or domestic service. But it was not always a fair exchange. Hospitals quickly recognized student labor as a cheap and easily exploited commodity. They structured training around their own staffing needs. Upon graduation, nurses found themselves in sex-stereotyped, overcrowded, and isolated practice, competing for jobs with poorly trained women and also with new nursing graduates.

Leaders among nurses -- most often the directors of large, prestigious, urban schools -- addressed these problems by attempting to professionalize nursing education and practice. By the early 1920s, nurses created the American Nurses Association (ANA), and successfullylobbied for strict registration credentials. However, the initial registration laws were voluntary.
It was not until the 1930s that registration became a legal prerequisite to practice. By that time, nurses themselves had actively proved their worth. Public health nurses, in particular, had moved aggressively to carve an independent domain of responsibility for the health care of immigrants and the working poor in urban areas. Funded by both private and public monies, these women brought health care to those people whom other clinicians were unable or unwilling to serve.

African Americans similarly supported their own community's nurses. Barred by racist custom and often by law from working in the U.S. health care system, Black communities established hospitals for their sick and offered nurses' training schools for Black women. These activities strengthened the reciprocal bonds between Black nurses and the people they served, despite the nurses' continued exclusion from the white-run health care system. African American nurses established an ongoing commitment to care for people of color, particularly in the South, where doctors were scarce and health care limited. These nurses garnered respect and a status within the Black community often denied their white sisters, although they earned less and worked under much harsher conditions.

In 1908 African American nurses, excluded from many of the state nurses' associations, organized the National Association of Colored Graduate Nurses (NACGN) to represent their interests and to push for full integration into nursing's social and professional life. Their moment came at the end of World War II when, in 1945, the army and the navy nurse corps opened their doors to African American nurses. In 1948 the ANA followed suit and in 1951 the NACGN disbanded. In 1971 the National Black Nurses Association formed, both out of concern that the ANA had failed to recognize the contributions of African American nurses and out of a vision to increase the numbers of and the opportunities for all nurses of color.

In the postwar era, rapid changes occurred in health care treatments, technologies, research, and financing. These changes redefined nursing practice. In the new intensive care units, for example, one's background and education (in nursing, as in other professions, code words for race and class) often mattered less than one's skill and competence. Nurses then began organizing in new ways. Clinical associations now promised to be a bridge of sorts among nurses from diverse backgrounds and with professional aspirations.Black Women in White: Racial Conflict and Cooperation in the Nursing Profession, 1890 - 1950, Darlene Clark Hine (Bloomington: Indiana University Press, 1989)Ordered to Care: The Dilemma of American Nursing, 1850 - 1945, Susan Reverby (Cambridge: Cambridge University Press, 1987)
Patricia D'Antonio

(c) Copyright 1998 Houghton Mifflin

Unknownnoreply@blogger.com0tag:blogger.com,1999:blog-503732791294005663.post-87152194270487675892008-03-02T01:03:00.000-08:002008-04-13T08:02:37.309-07:00Florence Nightingale -The Lady with the Lamp

Born

12 May 1820820-05-12)
Florence
Grand Duchy of Tuscany

Died

13 August 1910 (aged 90)
Park Lane, London, England.

Profession

Nurse

Institutions

Selimiye Barracks,Scutari

Specialism

Hospital hygiene and sanitation

Known for

Pioneering modern nursing

Notable prizes

Royal Red Cross
Order of Merit

Florence Nightingale (1820-1910), British nurse and hospital reformer who became internationally known as the founder of modern nursing. Herbert was already married, but he and Nightingale were immediately attracted to each other and they became lifelong close friends. Herbert was instrumental in facilitating her pioneering work in Crimea and in the field of nursing, and she became a key advisor to him in his political career. On 21 October 1854, she and a staff of 38 women volunteer nurses, trained by Nightingale and including her aunt Mai Smith,[5] were sent (under the authorization of Sidney Herbert) to Turkey, about 545 km across the Black Sea from Balaklava in the Crimea, where the main British camp was based.

Nightingale arrived early in November 1854 at Selimiye Barracks in Scutari (modern-day Üsküdar in Istanbul). She and her nurses found wounded soldiers being badly cared for by overworked medical staff in the face of official indifference. Medicines were in short supply, hygiene was being neglected, and mass infections were common, many of them fatal. There was no equipment to process food for the patients.

Florence and her compatriots began by thoroughly cleaning the hospital and equipment and reorganizing patient care. However, during her time at Scutari, the death rate did not drop; on the contrary, it began to rise. The death count would be highest of all other hospitals in the region. During her first winter at Scutari, 4077 soldiers died there. Ten times more soldiers died from illnesses such as typhus, typhoid, cholera and dysentery than from battle wounds. Conditions at the temporary barracks hospital were so fatal to the patients because of overcrowding and the hospital's defective sewers and lack of ventilation. A sanitary commission had to be sent out by the British government to Scutari in March 1855, almost six months after Florence Nightingale had arrived, which flushed out the sewers and improved ventilation. Death rates were sharply reduced.

Nightingale continued believing the death rates were due to poor nutrition and supplies and overworking of the soldiers. It was not until after she returned to Britain and began collecting evidence before the Royal Commission on the Health of the Army, that she came to believe that most of the soldiers at the hospital were killed by poor living conditions. This experience would influence her later career, when she advocated sanitary living conditions as of great importance. Consequently, she reduced deaths in the Army during peacetime and turned attention to the sanitary design of hospitals.

The Lady with the Lamp

During the Crimean campaign Florence Nightingale gained the nickname "The Lady with the Lamp", deriving from a phrase in a report in The Times:

She is a ‘ministering angel’ without any exaggeration in these hospitals, and as her slender form glides quietly along each corridor, every poor fellow's face softens with gratitude at the sight of her. When all the medical officers have retired for the night and silence and darkness have settled down upon those miles of prostrate sick, she may be observed alone, with a little lamp in her hand, making her solitary rounds.

The phrase was further popularised by Henry Longfellow's 1857 poem "Santa Filomena":

Lo! in that hour of misery
A lady with a lamp I see
Pass through the glimmering gloom,
And flit from room to room.

Return home

Florence Nightingale returned to Britain a heroine on 7 August 1857, and, according to the BBC, was arguably the most famous Victorian after Queen Victoria herself. Nightingale moved from her family home in Middle Claydon, Buckinghamshire, to the Burlington Hotel in Piccadilly. However, she was stricken by a fever, probably due to a chronic form of Brucellosis ("Crimean fever") that she contracted during the Crimean war. She barred her mother and sister from her room and rarely left it.

In response to an invitation from Queen Victoria – and despite the limitations of confinement to her room – Nightingale played the central role in the establishment of the Royal Commission on the Health of the Army, of which Sidney Herbert became chairman. As a woman, Nightingale could not be appointed to the Royal Commission, but she wrote the Commission's 1,000-plus page report that included detailed statistical reports, and she was instrumental in the implementation of its recommendations. The report of the Royal Commission led to a major overhaul of army military care, and to the establishment of an Army Medical School and of a comprehensive system of army medical records.

Later career

While she was still in Turkey, on 29 November 1855, a public meeting to give recognition to Florence Nightingale for her work in the war led to the establishment of the Nightingale Fund for the training of nurses. There was an outpouring of generous donations. Sidney Herbert served as honorary secretary of the fund, and the Duke of Cambridge was chairman. Nightingale was also considered a pioneer in the concept of medical tourism as well based on her letters from 1856 in which she would write of spas in Turkey detailing the health conditions, physical descriptions, dietary information, and other vitally important details of patients whom she directed there (which was significantly less expensive than Switzerland). She was obviously directing patients of meagre means to affordable treatment.

By 1859 Nightingale had £45,000 at her disposal from the Nightingale Fund to set up the Nightingale Training School at St. Thomas' Hospital on 9 July 1860. (It is now called the Florence Nightingale School of Nursing and Midwifery and is part of King's College London.) The first trained Nightingale nurses began work on 16 May 1865 at the Liverpool Workhouse Infirmary. She also campaigned and raised funds for the Royal Buckinghamshire Hospital in Aylesbury, near her family home.

Nightingale wrote Notes on Nursing, which was published in 1860, a slim 136 page book that served as the cornerstone of the curriculum at the Nightingale School and other nursing schools established. Notes on Nursing also sold well to the general reading public and is considered a classic introduction to nursing. Nightingale would spend the rest of her life promoting the establishment and development of the nursing profession and organizing it into its modern form.

Nightingale's work served as an inspiration for nurses in the American Civil War. The Union government approached her for advice in organizing field medicine. Although her ideas met official resistance, they inspired the volunteer body of United States Sanitary Commission.

In 1869, Nightingale and Elizabeth Blackwell opened the Women's Medical College.

In the 1870s, Nightingale mentored Linda Richards, "America's first trained nurse", and enabled her to return to the USA with adequate training and knowledge to establish quality nursing schools. Linda Richards went on to become a great nursing pioneer in the USA and Japan.

By 1882, Nightingale nurses had a growing and influential presence in the embryonic nursing profession. Some had become matrons at several leading hospitals, including, in London, St Mary's Hospital, Westminster Hospital, St Marylebone Workhouse Infirmary and the Hospital for Incurables at Putney; and throughout Britain, e.g. Royal Victoria Hospital, Netley; Edinburgh Royal Infirmary; Cumberland Infirmary; Liverpool Royal Infirmary as well as at Sydney Hospital, in New South Wales, Australia.

In 1883, Nightingale was awarded the Royal Red Cross by Queen Victoria. In 1907, she became the first woman to be awarded the Order of Merit. In 1908, she was given the Honorary Freedom of the City of London.

By 1896, Florence Nightingale was bedridden. She may have had what is now known as chronic fatigue syndrome. She made extensive use of statistical analysis in the compilation, analysis and presentation of statistics on medical care and public health.

Nightingale was a pioneer in the visual presentation of information. Among other things she used the pie chart, which had first been developed by William Playfair in 1801. After the Crimean War, Nightingale used the polar area chart, equivalent to a modern circular histogram or rose diagram, to illustrate seasonal sources of patient mortality in the military field hospital she managed. Nightingale called a compilation of such diagrams a "coxcomb", but later that term has frequently been used for the individual diagrams. She made extensive use of coxcombs to present reports on the nature and magnitude of the conditions of medical care in the Crimean War to Members of Parliament and civil servants who would have been unlikely to read or understand traditional statistical reports.

In her later life Nightingale made a comprehensive statistical study of sanitation in Indian rural life and was the leading figure in the introduction of improved medical care and public health service in India.

In 1859 Nightingale was elected the first female member of the Royal Statistical Society and she later became an honorary member of the American Statistical Association.

Literature and the women's movement

While better known for her contributions in the medical and mathematical fields, Nightingale is also an important link in the study of English feminism. During 1850 and 1852, she was struggling with her self-definition and the expectations of an upper-class marriage from her family. As she sorted out her thoughts, she wrote Suggestions for Thought to Searchers after Religious Truth. The three-volume book has never been printed in its entirety, but a section, called Cassandra, was published by Ray Strachey in 1928. Strachey included it in The Cause, a history of the women's movement. Apparently, the writing served the original purpose of sorting out thoughts; Nightingale left soon after to train at the Institute for deaconesses at Kaiserwerth.

Cassandra protests the over-feminization of women into near helplessness, such as Nightingale saw in her mother and older sister's lethargic lifestyle, despite their education. She rejected their life of thoughtless comfort for the world of social service. The work also reflects her fear of her ideas being ineffective, as were Cassandra's. Cassandra is a virgin-priestess of Apollo who receives a divinely-inspired prophecy, but her prophetic warnings go unheeded. Elaine Showalter called Nightingale's writing "a major text of English feminism, a link between Wollstonecraft and Woolf."[11]

Theology

Suggestions for Thought is also Nightingale's great work of theology, her own theodicee, where she develops her radical heterodox ideas.
